1. Complete online passport application form via (Apply for Renewal/Re-issue Passport)
2. Upload supporting documents;
3. Birth certificate issued by the National Population Commission or Declaration of age by Federal/State High Court or Customary Court of Appeal.
4. Local Government Certificate of Indigeneship.
5. Police extract for Loss of passport.
6. Sworn Affidavit for Loss of Passport from the High Court.
7. If you have Non-Nigerian passport and hold dual nationality, upload data page of your Non-Nigeria passport.
8. Additional supporting document (such as marriage certificate where applicant’s name is different from the other documents)
9. Pay the required fees for the replacement passport.
10. Print out Acknowledgement slip and payment slip.

Note: The continued use of a passport declared lost may be considered fraudulent use of a government document and could result in:

• Imprisonment for up to 10 years,
• A fine of up to ₦2,000,000, or
• Both imprisonment and a fine.

This is in accordance with Sections 10(1)(e) and 10(1)(h) of the Immigration Act.
